Muscles need oxygen. And that applies to both skeletal muscles and heart muscle.
When the body has enough iron ions, it can produce plenty of hemoglobin, the protein in red blood cells that carries oxygen in the blood. Iron is also a component of myoglobin, the protein responsible for intramuscular oxygen transport.
A lack of iron leads to impaired mitochondrial function (the cells' powerhouses). It's no wonder that fatigue is one of the first symptoms of iron deficiency. Iron deficiency can also be accompanied by a rapid heartbeat.
In patients with cardiovascular disease, optimizing iron status leads to improved cardiac function. Patients with heart failure, in particular, benefit from iron therapy; for these patients, target ferritin levels are above 100 ng/ml.
Measuring iron levels is useful for young women who experience monthly iron losses, for older people—women or men who have either an inadequate diet or poor intestinal absorption—and also for pregnant women, who have a "double" need for iron for themselves and their babies. Vegans may also be at risk of iron deficiency.
For women, ferritin levels >60 ng/ml are ideal, for men >120 ng/ml. Lower values, even if within the laboratory's reference range, are insufficient and should be corrected.
In cases of very low ferritin levels or if oral iron is not well tolerated, it is recommended to compensate for the deficiencies by intravenous iron administration, which is simple and risk-free.
